MCHD Persistent Pain and Opioid Management Workflows/Policies

Initiated as part of a project to expand access to Medication Assisted Treatment (MAT) in primary care, Multnomah County Health Department Primary Care Clinics have implemented updated workflows and policies for persistent pain and opioid management. The clinical workflows incorporate Providence’s Persistent Pain Toolkit, and include standardized guides for the management of different types of persistent pain patients, such as new patients on chronic opioids, existing patients on chronic opioids, patients starting opioids, and existing and new patients not on opioids. The policies and workflows also include EPIC smart phrases and tools to make their use with patients easier and standardize the documentation providers use in the electronic health record for managing patients with persistent pain.
